(1) I understand that to be accepted into the program, I must complete this application, pay $35 to enroll, and pay for the first 2 weeks of afterschool service.
(2) I understand that my child may continue receiving afterschool services if I make advance payments every 2 weeks by Children of the City's payment deadlines.
(3) I understand that I will receive invoices and reminders at the email address I have provided in this registration form.
(4) I understand that I can discontinue afterschool services at any time.
(5) I understand that my payments are non-refundable.
(6) I understand that I must pay the full biweekly rate unless there is a school closure, holiday, or prolonged sickness.
