Keep in mind that completing this form does not guarantee a spot in the classes that will begin July 8, 2019, due to limited space. Final confirmation of registration will be via an email and after payment is made or arranged for.

PLEASE BE AWARE THAT REGISTRATION IS NOT FULLY COMPLETE AND FINALIZED UNTIL PBS RECEIVES EITHER PAYMENT BY CHECK OR EMAIL CONFIRMATION FROM THE 3RD-PARTY PAYER THAT AN INVOICE FOR THE CLASS(ES) WILL BE PAID.

IN THE EVENT THAT SOMEONE REGISTERS AFTER THE CLASS IS FULL, THEY WILL BE NOTIFIED BY EMAIL AND PLACED ON A STANDBY LIST.
