By signing below, I authorize that my child has permission to attend and self dismiss from the Hoboken City Wide All Schools 7th grade dance at Hoboken High School on May 13, 2022 from 7:00pm-9:30 pm. This signature from the parent/guardian is acknowledgment and acceptance of this release of liability. Please note, by signing below, the parent/guardian releases the volunteers, Hoboken Board of Education, and the Hoboken Public School District from any and all liability associated with the child leaving the 7th grade Hoboken All Schools City Wide dance on Friday, May 13th, 2022 without a parent/guardian or other authorized adult. By signing below, I understand this is a parent run/volunteer event. The City of Hoboken, Hoboken Board of Education, and the Hoboken Public School District are not involved or responsible for the planning and event. Due to the safety of the children, no adults will be allowed inside the Hoboken High School except for the approved chaperones. If you would like to pick up your child, you will be permitted to enter the Hoboken High School at 9:30pm. We appreciate your understanding in this matter.
