Thank you very much for participating in the GeoCorps America program! Please tell us about your experience by completing this evaluation shortly after the end of your project. You do not need to share your responses to this evaluation with your mentor, and GSA will not share your responses with your mentor, unless you specifically indicate that we may. Your responses will be used to guide improvements to the GeoCorps program. Any feedback given to mentors will be generalized and remain anonymous.

This evaluation form is due to be received at GSA by 12:00 noon (Mountain Time) on September 1, or 10 days after the end date of your GeoCorps project, whichever comes later. This form must be received by the deadline in order to receive your completion certificate.

Please answer each question honestly and to the best of your ability. Your feedback is very important to us.

This evaluation should be completed by GeoCorps America participants whose projects were hosted by the US Forest Service (USFS).

If your GeoCorps America project was not hosted by the USFS please see the GeoCorps America website (www.geosociety.org/geocorps) for the survey for the organization that hosted your project.
