1. Xiao En Counselling & Support provides the following services on the objective to provide mental health support for the public.　　
• Individual Counselling
• Duo/ Couple Counselling and Family therapy
 
2. All services are provided by appointment only. Reservations can only be made through online booking. On-site appointments are not accepted.

3. This form is the first step of the booking procedures. You are required to fill in this form and these information would help us to understand your situation and needs.

4. This form has the following 5 parts, it takes about 6-8 minutes to complete.
    (1) Introduction to Counseling Services        
    (2) Personal Information
    (3) Presenting Issues
    (4) Physical & Psychological Status (within 2 weeks)
    (5) Booking

5. If this form is not completed in accordance with the instructions, the appointment will be deemed invalid automatically. If the appointment is unsuccessful, there will be no further notice, please make a new appointment by yourself.

6. All counselling services are charged. After payment, the fee will not be refunded or transferred in the event of a missed appointment, cancellation of the appointment, or invalidation of the appointment.

7. Xiao En Counselling & Support has the right to modify, amend, add on or delete any terms and conditions anytime as well as cancel, suspend or terminate this service anytime without notice.
