For consideration of requests for use of the church facilities, this form must be completed in its entirety. A rental agreement will be presented based on the requested day(s) and time(s) along with amount due. Please note, if requesting a minister we must coordinate both venue and minister availability.
All payments shall be received within 7 days of the event unless otherwise noted in invoice.
