Liability Release

By agreeing below, I the parent/guardian of the player above acknowledge and accept all conditions of Great Lake Canadians hosted events.  I hereby give permission to the Great Lake Canadians and all of their staff and personnel to act in the best interest of my child should an emergency, medical or otherwise arise in my absence.  Furthermore, I release the Great Lake Canadians and any of their staff or personnel from any claim arising from my child’s participation in Great Lake Canadians sanctioned events. 

I further acknowledge that my child is in good health and has no medical conditions that should preclude him from participating in a workout of this nature.  I’m allowing the player to participate in baseball related activities, and agree fully and forever release, discharge, hold harmless, Great Lake Canadians Baseball Inc. all associated employees, owner and agents from any and all claims, demands, damages or rights of action, present or future resulting from any players participation in our events.
