By completing this form, you agree and understand: that base rate for a commission is listed by the size (includes shipping), to be adjusted based on the specific level of detail required in the order. You agree to paying a 50% deposit on agreement of purchase prior to Patrick beginning work, and to pay the final 50% upon completion, but prior to delivery/pickup/shipping. You also understand that while you own the original, Patrick Guindon Art owns the copyright and can reproduce/license/use the work in whatever format he feels suitable for future use, including prints, notebooks, cards, marketing, and other designs. *
