NNHS Transcript Request for Graduates/Former Students
Please use this form only if you are a former student of Newton North High School. If you graduated more than five years ago, there is a $5 fee for transcript requests. After completing this form, please mail a check to  Newton North High School, Records Office, 457 Walnut Street, Newtonville, MA 02460.  Checks should be made to the City of Newton.  Please indicate if this is a hardship in the space provided below.

If you graduated less than five years ago, your fee is waived and you may submit this form with no further action. If you would like to request the counselor statement and teacher recommendations to be sent to an academic institution, you can make that request on this form as well.

IMPORTANT NOTE:  The official transcript is a document with the school seal and principal's signature. The official transcript is sent directly to institutions (college, university, employer) only. The unofficial transcript, or student copy, can be sent directly to students via postal mail, fax or email.  Official transcripts cannot be sent directly to a former (or current) student.
