This form is designed for Affiliates to submit players from within their association to participate at the 2020 Vawdon Cup, as their association is not entering teams in the respective division the player wishes to participate in.

For further information on the process undertaken here please refer to the 2020-21 NSWTA General Conditions Of Entry - Clauses 3.1.9 through to 3.1.13.

ONLY an Affiliate Coordinator of an association can complete this form on the players behalf. No player can complete this form.
