This is the official entry form for Filmit 2021: A Student Film Competition. Please complete all sections relevant to your entry. In Section 2, the Entry Submission Area, it is important that you paste the link to your Google folder that contains the following documents:
■ Entry Form (School name_Entry Form);
■ Use of Images Permission Form (Parents) (School name_Parent Permission Form);
■ Use of Images Permission Form (School) (School name_School Permission Form); and
■ The video (School name_Video title).
■ The poster design, if relevant (School name_video title_poster)

Section 3 requires the names of the students involved in making the film. Please write them individually in the specific order as shown in the relevant box.

Section 5 requires the names of the students involved in creating the poster. If you have NOT submitted a poster entry, please indicate 'No' in Section 4 .
