Hello! Thank you helping fill out this feedback form regarding student experience relating to equity, diversity and inclusion (EDI) in DCS clubs.
The purpose of this form is to gain feedback from students involved in specific clubs so that the DCS EDI team can create an individualized action plan for club executives using the information collected.
We understand conversations regarding EDI, specifically experiences of discrimination or microagressions, can be very sensitive and personal to those involved. The information collected in this form will remain confidential and anonymous and will only be viewed by select DCS Executive and the EDI team. To encourage honest feedback, the information shared will not be be directly viewed by club executives.
