The following information is to be used for informational purposes for enrollment into the Salt Lake School for the Performing Arts. By completing this application it gives SPA staff your interest, and intent. 

Please know you will receive an email from our schools enrollment secretary with more information and possible acceptance. You must complete this form to be considered for enrollment.

We highly encourage all prospective students to tour the school and or participate in one of Shadow Days.
