LabGram Order Form
Thank you for your interest in MadLab's LabGram - an ongoing fundraiser where people can purchase personalized poems or parodies! That's a lot of great P's! Each poem/song parody will be personally written and dramatically performed by one of our talented actors! The LabGram will be put into video format and delivered to you via email, so you can send it to the recipient of this unique, personalized gift. To help us write this, we need to get some information about your loved one. But first, funds.

The minimum recommended donation is $10-$15.
After receiving this completed form AND your donation, we will begin working on your LabGram. You can expect your LabGram to be delivered to the email provided on this form, roughly 7 days from receiving this form and  your donation.

If nothing comes to mind, no worries, we'll pick a song for you.
