Congratulations! You are eligible to apply for membership in Lake Zurich High School's chapter of Tri-M Music Honor Society. Only the top music students, both academically and by virtue of performance, are selected. Please answer the follow questions, having given some thought to your answers - no research is needed, but they should be given adequate consideration (i.e. no short answers here!) Due MAY 6th!

What Musical activities do you currently participate in, both in and out of school? What will you be participating in this coming year?
Members in Tri-M must fulfill the following obligations to the group::
a. Participation in two LZHS music ensembles OR an ensemble and sanctioned performance group OR an ensemble plus a sanctioned community music ensemble.
b. Maintaining an A average in all music classes and an overall B average (3.0 on a 4.0 scales).
c. Remain off of the ineligible list.
d. Attending monthly Tri-M meetings on the 3rd Thursday
e. Participate in the many service projects throughout the year.
f. Attend high school concerts (besides your own) throughout the year and serve as an usher.
g. Perform a total of 10 hours of service (2 of which is the ushering at school concerts)
