Completing Forms Acknowledgement *  I understand to be eligible to participate in tryouts all forms located at http://podcasts.shelbyed.k12.al.us/hlhs-athletics/coach-parker-helena-athletics/ must be completed and turned in. This includes the concussion information form, emergency information and consent form, physical form, student release form, star sportsmanship completion certificate, volunteer driver form, drug form, and HHS Athletic Handbook. There are various methods to turn these in. *
