ما هو الخطأ المزعوم الذي تبلغ عنه؟  صف ما حدث، بما في ذلك أكبر قدر ممكن من التفاصيل What behavior do you want to report? Describe what happened, including as much detail as possible
من ارتكب المخالفة المزعومة؟  هل كان هناك أي شخص آخر متورط؟  قدم الأسماء الكاملة والألقاب إن أمكن Who you report about? Was there anyone else involved? Provide full names and nicknames if applicable
متى وأين حدث ذلك؟  حدد التواريخ والأوقات، إذا كانت متوفرة When and where did it happen? Specify dates and times, if available
