Chisago Lake Lutheran Sunday School Registration
We're excited to have your kids learning and growing in the love of Christ this year!  To help us prepare and have enough volunteers, please fill out this registration form for your family.  Sunday School is for preschool up thru 5th grade.  Sunday School begins with Rally Day on September 10th.  Worship is at 9 am, with Back to school Blessing during worship, followed by Games, Bounce House, Donuts, Cotton Candy & Popcorn!!!  Questions?  Talk with Stacy at 257-6300 or 218-330-9125.

Please Note:  All children attending Sunday School should be 3 years old (and potty trained) by September 1, 2023

This registration form covers all events and activities included in Sunday School and Children's Ministry Activities.  Separate permission slips are not needed, except for off site activities.

I authorize my child to attend Chisago Lake Lutheran Church (CLLC) events during the 2023-2024 School Year.  In the event a medical emergency arises and the emergency contact or I cannot be reached by phone, I authorize Chisago Lake Lutheran Church and its agents to provide for my child.  I further release Chisago Lake Lutheran Church as well as all of its agents, members or employees, for all liability for any accident, injury or claim arising from my child’s use of its facilities, transportation or participation in any of its programs. Furthermore, I take full responsibility for any financial cost, which may be incurred, for the care of you child, including emergency transportation and hospitalization.  I release Chisago Lake Lutheran Church and all of its agents from any liability for lost, stolen or damaged articles.   I understand photos may be taken and used for church purposes only.  I also give consent for CLLC staff & volunteers to use photos taken of my children during any activities or events for promotional materials & to be placed on the church website.  
NOTICE OF FILMING/PHOTOGRAPHY:  CLLC will be filming/photographing on occasion throughout the year.  By attending CLLC programs, you irrevocable consent to and authorize CLLC to photograph and make audio and visual recordings of your child and grant CLLC the right to use photographs and recordings for the purposes of CLLC.  I understand that as a precaution, my child/ren’s names will NOT be published or linked with photographs.
