Please read carefully. No exceptions are allowed. This is a process and FOA follows it and makes no allowances for anyone.  If you have a stray you will need to contact the authorities as this is for owner surrender only.

Veterinarian Records must be from your vet’s office; your records WILL NOT suffice.

1. You will need to sign a records release at your vet’s office for FOA to receive all records for the animal
2. You will need to sign a release at your vet’s office for FOA to speak with all staff and veterinarians for the animal
3. You will need to have your vets records emailed to friendsoftheaniamls52772@gmail.com 

All records can be emailed to FOA and need to be received within 5 DAYS from application date. If not received on the 6th day FOA will shred the application and you will need to restart the application and process.

FOA DOES NOT OBTAIN YOUR VET RECORDS FOR YOU. YOU WILL NEED TO DO THIS STEP. NO EXCEPTIONS.

Please note in the SUBJECT LINE of the email:

1. OWNERS NAME ON THE APPLICATION
2. DATE OF THE APPLICATION
3. ANIMALS NAME ON THE APPLICATION

We are not responsible for records that do not match up as we get multiple requests a day.

We allow 5 days to submit the records, but please understand we will not be able to talk to you and/or take your animal until the records are received and our intake coordinator has had time to review the records. FOA reserves the right to decline intake/surrender. FOA may at their discretion refer you to another organization if we feel the animal isn't a fit for our organization. FOA does not guarantee the referral will take the animal.

Please note: If the records not accompanied at the time of this application, we cannot take your word the animals are up to date on shots and/or spay or neutered as the state will not take our word. We have state and facility guidelines we must follow and hope you understand.


The surrender fee will be determined once the Intake Coordinator has looked at all records. FOA will contact you via email to set up a date and time for you to bring the animal to the facility for an interview and check the animal over before ANY final decision is made. Please leave your animal in the vehicle until you check in with the Intake coordinator as we do not allow them in the facility until they are looked over.

The email to set up a date and time for the interview and checking the animal over will contain the intake/surrender fee. The fee payment method is CASH and will need to be brought to the appointment. IF the animal is accepted, we will collect the intake/surrender fee and be retaining the animal at this visit.

IF FOA accepts the animal PLEASE NOTE

1. Intake/Surrender fee is due in CASH at the time of surrender. You will know the amount before you come in for the interview and our Intake/surrender coordinator to check the animal over.
2. A signature is required on the Intake/surrender paperwork
3. A signature is required for no contact etc. form
4. A veterinarian release for your vet is required for 1 yr.
5. If you purchased your animal from a breeder, we require a release to speak with them
