Sacrament of Jnr. Reconciliation and Eucharist
Dear parents and carers,
To register your child for the Sacrament of Jnr. Reconciliation and Eucharist, please complete the following form. If you have any trouble contact Lia at ballaratnorth@ballarat.catholic.org.au

As a reminder, your child's attendance with a parent or guardian is required for the Monday evening preparation sessions (5:45pm - 6:30pm), as well as the Sacrament of Reconciliation (you have a choice, see below) and the Sacrament of Eucharist (you have a choice, see below).
Your child's preparation will be enriched by their involvement in weekend Mass, you may choose to attend on Saturday evening at 6pm at Our Lady Help of Christians Wendouree or 10am on Sunday morning at St Columba's Ballarat North.

To secure your registration, please drop of the $30 registration fee and your trifold (not your stole) to the St Columba's Primary School office.
