This form is for students who wish to borrow a school instrument for the school year. This form must be completely filled out.  It is HIGHLY recommended if students can afford to do so, to rent an instrument from one of the companies we trust (Long & McQuade or Tapestry Music). School instruments are intended for families who may require financial assistance. Students requesting a school instrument are asked supply $40 to cover maintenance and repair costs for the year. Once this form has been completed, and $40 hand delivered to either Mr.Kynoch or Mr.Gaucher, the instrument will be distributed to the student. Please bring the money in an envelope clearly marked with the students first and last name. If paying with a cheque, please make it out to "ADSS" or "ADSS Band".

*Students wishing to borrow a secondary instrument for Jazz Band may do so at no additional cost. Students will only be allowed to borrow ONE additional instrument.

**Any instruments extremely damaged or stolen will be charged to a student's account at the school and all expenses required to be paid by the parent/guardian. By filling out and completing this form you are agreeing to these conditions
