I authorize the University of Alaska to release my academic and financial aid information to the scholarship committee for the purpose of scholarship consideration. This application in no way guarantees funding from the UAS Sitka Campus. 

I am responsible for all charges to my student account including all tuition, fees, textbooks or material expenses.

I must have a passing grade at midterms, and I acknowledge that any funds that may be applied to my account will be contingent on a midterm grade of C or higher.

No funds will be applied to my account until after midterm grades and the scholarship committee have determined that I am eligible.

By dropping or withdrawing from a course that may qualify for tuition assistance, I will no longer be eligible to receive funding for that class.
