The child listed on this form has
my permission to participate in Day Camp 2024. I understand that he/she must follow the camp
rules. The Camp Dean has the right to
dismiss any child who does not follow the camp rules, in which case, I will
have to come and pick up my child. I
release and agree to hold blameless Clover Road Christian Church and all camp
volunteers from any and every claim arising, or which may be asserted by me or
by any member of my family. I authorize
the camp dean, in the event I cannot be reached by phone, to give consent to a
physician and/or hospital for emergency medical or surgical treatment while at
camp. It is understood that I will
assume any financial responsibility for any expense incurred for the treatment. I do certify that my child is covered by
insurance. My child can be included in
the camp video and pictures taken at camp.
