Please use this form to register for our LGN - South FTW membership. Contact information entered here will be used for directory and shared with team. If your billing info is different please let us know via email.

Membership dues are $50/mth and includes a one time $100 application fee (non-refundable). Once you submit this form you will receive an invoice for payment to complete the registration.  

Registration is not complete until payment has been received.

Do you accept our general rules?

- I will setup Autopay to pay member dues by 1st of every month
- I will attend weekly and show up on time.
- I will have a substitute if I cannot attend.
- I understand if I miss three meetings in a three month period, my membership may be cancelled after review.
- I will show up positive and ready to contribute.
