Member Suspension Form  
We understand that you would like to temporarily suspend your membership at 423 Yoga. Please fill this short form 14 days prior to the suspension date (for medical emergencies, suspension can be immediate) and we will be sure to freeze your membership for the requested amount of time. Please be aware that suspensions are for a minimum of 4 weeks and a maximum of 3 months (can be extended by sending form again); a return date is required. Suspensions are only permitted one time per 12 months. thank you
