Please use this form when you have uniform donations in order to be entered into our incentive raffles.   Please be sure to CLEARLY label your donations with the name you are using on this form so that we may match your donation to the form.    If we do not receive your donation to the K-4 front desk within 5 business days of submission, your name will be removed from the raffle drawing and your submission deleted.  You may resubmit once you are ready to make your donation.   You do NOT need to print the form to attach to your donation, you only need to clearly label the bags with the name used on this form and then submit this form online.
