This form is to be completed only ONCE for each group. Choose a group leader to sign up your group.
**Middle School students may only be in groups with other Middle School students. The MS chaperones will be assigned to chaperone the MS group their child is in.
**High School students may only be in groups with other High School students. The HS students will only report to the HS chaperones via text every hour on the hour. The HS chaperones will be assigned to the HS group their child is in. Whether they travel with the HS group or not is up to them. HS chaperones are welcome to form their own groups if they prefer not to travel the parks with their kids.
All traveling groups will be a minimum of FOUR students and a maximum of EIGHT.
HS groups can travel with other HS groups at the parks, but students must stay with their assigned groups at all times.
You may sign up a group of 2 or 3 students, but KNOW that your small group will be ADDED to another group in order for the minimum of 4 to be honored.
Any individual students not signed up in any group will be placed in another group after the deadline for this form passes.
