***This form is to be filled out BY THE SPONSOR following a conversation with the Candidate about why the Candidate chose to ask him or her (the candidate may assist in completing this form where necessary, due to technological challenges, etc.).***

A Sponsor must be a fully initiated Catholic in good standing, an active member of a Catholic parish, at least 16 years old at the time of the Confirmation liturgy,  and may NOT be a parent of the candidate.

If the Sponsor is NOT an active member of St. Bernadette Parish, please have the Sponsor's parish send a Sponsor verification certificate or letter of good standing.
