Student/Parent iPad Agreement
This form is required to be completed for a student to use a district issued iPad. A form must be completed for each child.

Please review the ISD 318 1:1 Digital Student Learning Initiative policies. These are located online through the district web page www.isd318.org > Menu > Services. >Technology >Technology Forms and Policies.

The iPad remains the property of ISD 318. At the end of the school year or upon transfer from the district, parents and students agree to return the iPad, Apple iPad Wall Charger, and Cord to their school in the same condition it was issued to the student less reasonable wear.
