The undersigned Team applies to volunteer for the Bayanihan Feeding Program at Binhi At Ani Filipino Community Center.

1. All Team volunteers must sign and comply with the Volunteer Agreement. *
2. The Team will cook at least 150 plate lunches and the plate lunches must be ready for delivery to the designated locations no later than by 11:30 a.m. Binhi at Ani will provide the instructions for delivery (location, contact person and information, number of plate lunches to deliver). Team will have access to the kitchen no earlier than 6:00 am. *
3. The Team will assign two adult drivers to deliver the plate lunches. The Team captain will check each driver has a) a valid driver’s license and b) a valid insurance card. *
4. There will be on-site at all times at least one Team member with a food handler permit.  The Team will comply with all food safety rules and regulations and all Binhi at Ani rules and regulations. *
5. At least thirty (30) days in advance, the Team will provide a wish list of food items for Maui Food Bank and/or Binhi at Ani to provide. If such food items are not available, the Team will be informed. The Team may also volunteer to provide their own food to cook, without any reimbursement. *
6. Maui Food Bank and/or Binhi at Ani will provide the necessary supplies (plates, utensils) but the Team may also volunteer to provide such supplies, without any reimbursement. *
7. If there is a replacement of the Team Captain, Binhi at Ani must be immediately notified. *
8. If the Team is not able to fulfill its schedule, the Team must find a replacement, which complies with all requirements. *
