Annual Membership Registration for Miracle Bonus Mothers of Multiples Club
Please completely fill out the form below. Complete registration forms are needed for each member each year to validate our records. Please help us in this effort to keep our records up-to-date. Birthdays and anniversarys will be used in our club newsletter. Thank you

ALL Miracle Bonus Club dues are due at the June meeting (2nd Monday of the month at 7pm).
If you cannot attend the meeting, please mail the form and a check (mail by June 15th) to:

Dues are $25 per year. Please make your checks payable to: Miracle Bonus.
Membership is offered FREE to all expectant moms.
Triplet mom’s will get 1 year free.

When you join Miracle Bonus, you will automatically become a member of the Northwest Association of Mothers of Twins Club (NWAMOTC). You will also receive their publication, “Twin-O-Gram,” four times a year. As a club member, you will also receive our monthly newsletter, and vote on club issues. Your $25 annual dues help fund our newsletter, rental of our meeting place, convention , and our Campership and Scholarship programs among other things.
