Machine Learning survey

This project is a part of the Master's program Data-Driven-Design at the HU University of Applied Sciences Utrecht in the Netherlands.

My name is Kiara Angelino, machine learning researcher on anterior cruciate ligament injuries. This research is being conducted and supported by TheKnee Club.

The project aims to indicate the risk and success factors of anterior cruciate ligament injury and surgery.

I welcome all participants that have tore their anterior cruciate ligament after 2019. It is crucial that participants meet this prerequisite. If you do not I thank you for continuing as far. Most questions are centered around sporting activities or very active sporters.

For best results we need you to not be distracted when answering the questions. Completing the questionnaire will take approximately 15 - 20 minutes.

Participants acknowledged and approved to have voluntarily contributed to this research. This research is taken anonymously and no personal information is stored. Unless you indicate this for in-depth contribution and or you want to stay informed about the results.

We thank you for your time and contribution to this research.
