𝐓𝐡𝐢𝐬 𝐫𝐞𝐪𝐮𝐞𝐬𝐭 𝐟𝐨𝐫𝐦 𝐢𝐬 𝐟𝐨𝐫 Crossroads 𝐬𝐭𝐮𝐝𝐞𝐧𝐭𝐬 𝐰𝐡𝐨 𝐰𝐨𝐮𝐥𝐝 𝐥𝐢𝐤𝐞 𝐭𝐨 𝐫𝐞𝐪𝐮𝐞𝐬𝐭 𝐚 𝐂𝐡𝐫𝐨𝐦𝐞𝐛𝐨𝐨𝐤. 

Students who complete this form prior to Monday, August, 2nd can expect to receive a Chromebook during open house on Monday, August,1st at 7pm.  It is the responsibility of the student to visit the Media Center to receive a Chromebook after completing this form. This form is only for Crossroads students. Be sure to only submit one form per Crossroads student; multiple forms for one student will slow the distribution process.

In completing this request and receiving a Chromebook, you agree to maintain the Chromebook and return it to the district in the same condition in which it was received (𝑷𝒍𝒆𝒂𝒔𝒆 𝒓𝒆𝒇𝒆𝒓 𝒕𝒐 𝒕𝒉𝒆 𝒔𝒕𝒖𝒅𝒆𝒏𝒕/𝒑𝒂𝒓𝒆𝒏𝒕 𝒉𝒂𝒏𝒅𝒃𝒐𝒐𝒌).
 Damages to a school issued Chromebook will be assessed at a reasonable repair cost up to and including full replacement cost if the device cannot be repaired. The full replacement cost at this time is approximately $360.

*If you find any issues with your assigned Chromebook, please bring it back to school and let the teacher know within the next 2 school days after receiving the Chromebook so that it may be switched out with no questions asked.  In future, if you have any issues with your Chromebook, please bring the Chromebook and the charger to Mr. Faith ASAP.
