Support Group Facilitator Survey
Please share your feedback as openly and honestly as possible so that we can make sure to deliver the best curriculum possible for the specific needs and desires of our LGBTQIA+ community members.

There are 26 REQUIRED questions, 23 of which are multiple choice. The other questions are optional but VERY helpful if you're willing to share more in depth.

This survey takes approximately 5 minutes to do the required questions only. However, we would really appreciate your elaborated and complete response.

NOTE: All of your information and responses will be kept strictly confidential. We will not share any personal information outside of the AFT team without your expressed written consent.

The data gathered in this survey will be used anonymously to address the needs of our community not currently being met by our local LGBTQIA+ organizations and help us advocate for more complete resources for our community, especially for transgender individuals including the resources that AFT provides.
