This application form is used to nominate a Casey Basketball Association for Life Membership. Life membership nominations are submitted to the Casey Basketball Board and are voted on by the executive committee.

These nominations are sent to the President and the Secretary.

Life membership is presented at the CBA Annual Awards Gala.

All nominations must be submitted by 31st December.
