Griffin Middle School Connections Course Request Form for the 2021-2022 School Year
Students should complete the following connections request form with an adult family member. Efforts will be made to accommodate all student requests, however courses are filled on a first-come, first-served basis and some courses require instructor approval. The Connections Course Request Form will close on May 21, 2021.

7th & 8th Grade Only: Is the student interested in continuing in Band, Chorus, and or Orchestra? Check the applicable answers.                                              Students can't be removed from Band, Orchestra, or Chorus without talking to the the Band teacher (Ashley.Cole@cobbk12.org), Orchestra teacher (Selinda.Proctor@cobbk12.org), or Chorus teacher (Sarah.Rabun@cobbk12.org).                                            6th grade: During the first week of school, sixth graders will participate in a fine arts showcase where they will be introduced to Band, Chorus, and Orchestra. They will make their decisions regarding fine arts on August 6, 2021.
8th Grade Only: If the student is interested in taking year-long, high school-level Visual Arts Comp. or year-long Business and Technology, please indicate it below. The student will also need to email the course teacher ( Heidi.Wong@cobbk12.org - Art; Sarah.Lyons@cobbk12.org - Business) to seek approval and complete an application.
