Failure to do so will result in the denial of access to the club.

The Club is permitting play based on the assurances that you have provided. Your signature on this document indicates your understanding of the inherent risks associated with the following activities; the game of golf, use of golf equipment, use of club facilities, participation in activities such as food & beverage services, and any other activities provided by the Club.
