By submitting this RSVP form and attending dance classes at Lisa's Dance Connection, you agree to the following release of liability.
This agreement is made this date submitted form in the City of Temple, County of Bell, State of Texas, between Lisa's Dance Connection and "A". Lisa Shed is engaged in the business of instructing the art of dance and maintains a studio at 2808 Cottonwood Lane, City of Temple, County of Bell, State of Texas. Parent, guardian, or adult student ("A") by executing this agreement hereby employs Lisa Shed or any faculty and assistant as the said Lisa Shed may designate to teach said forms of dance to "B" (Student/Students) and further agrees that the said Lisa Shed and her employees shall not be responsible for any injuries sustained by "B" during said dance instruction and while on the premise of said studio. "A" by executing this agreement certifies that "B" is at this time in good physical health and does not suffer from any physical abnormalities other than those set out and made part of this agreement. The parties hereto agree that it is their intention and covenant that this agreement and performance hereunder, and all suits and special proceedings hereunder be construed in accordance with and under and pursuant to the laws of the State of Texas which shall be applicable and shall govern to the exclusion of the law of any forum without regard to the jurisdiction in which any action or special proceedings may be instituted. In addition to the agreement listed above, I "A" am the parent/guardian of "B", a child/children under the age of (18) eighteen, and I hereby give my permission for said child/children to take instruction from Lisa Shed and her employees and agree to release Lisa Shed and her employees from all liability for any and all injuries suffered by my child under instruction or supervision at the dance studio.
