High School Dual Enrollment Application
In order to complete this application you must have ALREADY
1.  Completed an application of eligibility with your school counselor
2.  Completed your Shadow Visit at FMTC
3.  Completed and returned the paperwork that you received at your Shadow Visit with your parent.
4.  Provided test scores demonstrating Basic Skills Requirement
5.  Have on hand the FMTC Student ID number provided by Ms. Petrie

If you have not completed ALL of these steps - you are not yet eligible to complete the enrollment process.  Please contact FMTCAdmissions@leeschools.net to finalize the process.
