Jos hakemusta on tekemässä yhdessä useampi kuin yksi taho, kirjoittakaa tämän lomakkeen "Järjestön nimi" -kohtaan vain sen järjestön nimi, joka hakee avustusrahat ja toimittaa raportin ISYYlle. Muut yhteistyötahot tuodaan ilmi lomakkeen perustelukohdassa sekä liitteenä lähetettävästä hankesuunnitelmasta. Hankesuunnitelma, joka sisältää budjetin lähetetään liitteenä sähköpostitse viipymättä tämän lomakkeen lähettämisen jälkeen ja se tulee nimetä Hankeavustus_liitteet_JärjestönNimi.

If there is more than one organization applying for the grant, write on this form on the "Name of the organization" only the name of the organization that will use the project grant and will provide the report to ISYY. Other parties involved in the project can be named in the reasons for application part of the form and in the attachment project plan.
