Pueblo Community College Concurrent Enrollment Form
Students who are interested in participating in concurrent enrollment, must submit this form. 
Steps to apply for concurrent enrollment courses are as follows:

1.  Contact WAC School Counselor. - Review grades with the counselor to ensure that you have an 80% average grade at WAC from the prior semester, or have a 3.0 from your former school. (GPA of 2.5 is required for the CTE program at Pueblo Community College.) See this website for additional information from Pueblo Community College about concurrent enrollment.

2. Complete WAC’s Concurrent Enrollment Request Form.

3.  Apply Online to enroll at Pueblo Community College Obtain Student ID (S#) and login. It is free to apply.

3.  Apply for College Opportunity Fund

4.  Prep for & take NextGen ACCUPLACER Exam

5.  Submit CE Online Application. 

6.  Watch for an email asking you to sign your application.

7. Meet with the Pueblo Community College counselor and select courses that fulfill WAC's Graduation Plan and a program at PCC.

8. Once you have registered for courses, send a detailed Pueblo Community College schedule to WAC’s Registrar. This may be copied from your Pueblo Community College student portal

9. Upon completion of the semester, send a photocopy of all final grades to WAC's registrar to add to the WAC graduation plan to receive credit towards high school graduation.

Students must be in WAC’s 11th or 12th grade or have approval from WAC’s academic counselor to apply.

Students must be on track for high school graduation according to WAC’s Graduation Plan.

Students must have an 80% average grade at WAC from the prior semester, or have a 3.0 from their former school. - We want to make sure that students who are placed in college courses are prepared and ready for the exciting opportunity and challenge. 

Students may take up to 3 concurrent enrollment courses/semester. For additional courses, students must have approval from WAC’s academic counselor.

Tuition is paid by WAC for concurrent enrollment courses at Pueblo Community College.

I understand that WAC will not pay for texts or other fees associated with concurrent enrollment courses.

I understand that it is my responsibility to meet with Pueblo Community College's counselors to select courses. I must send WAC's registrar a screen shot of the courses that I registered for each semester. I also understand that it is my responsibility to submit a screenshot of my grades at the conclusion of each semester in order to receive credit towards WAC's Graduation Plan.

CONCURRENT ENROLLMENT/EARLY COLLEGE 

In-Person Activities Waiver

Concurrent Enrollment is an opportunity for qualified Williamsburg Academy of Colorado students to take college courses while in high school and receive both HS and college credit towards industry certificates, 2-year degrees, and transfer to 4-year universities. College courses are available on college campuses, and as such there are potential risks, including without limitation:

Students will be attending classes, and will be on campus with adults that have not had criminal background checks reported or monitored by WAC. 

Parents will be responsible for the transportation of their students to any and all classes or events associated with concurrent enrollment.

NOTICE: THIS IS A LEGALLY BINDING CONTRACT. In consideration of Leadership Academy of Colorado/Williamsburg Academy of Colorado, its agents, officers, volunteers, participants, employees, affiliates and all other persons or entities acting in any capacity on its behalf (collectively referred to herein as WAC), I hereby agree on behalf of myself, my children, my parents, my heirs, success, assigns, personal representatives and estate as follows:

I HEREBY ACKNOWLEDGE THAT THIS ACTIVITY ENTAILS KNOWN AND UNANTICIPATED RISKS, which could result in physical or emotional injury, paralysis, death, or damage to myself, to property, or to third parties. I understand that such risks simply cannot be eliminated without jeopardizing the essential qualities of this activity. The risks may include injuries from, among other things, transportation to, from, and during the activity, getting lost, and other hazards. During an activity there may be contact with adults that have not had criminal background checks reported or monitored by WAC.  In addition to the risks listed above, there may be risks specific to this activity, which I accept as they are inherent to this activity. I VOLUNTARILY ASSUME ALL RISKS WITH FULL KNOWLEDGE AND APPRECIATION OF THE DANGER AND RISK INVOLVED.

I release and discharge WAC from any claims or liability for any injuries, death, disabilities or property loss or damage I may sustain while participating in this activity with WAC even if arising out of the negligence of WAC. This release, however, does not extend to loss or damage arising out of intentional acts or from the gross negligence of WAC. I covenant not to make or bring any such claim against WAC, and forever release and discharge WAC from liability under such claims.

I hereby voluntarily agree to indemnify and hold harmless WAC from any and all claims, demands, or causes of action, which are in any way connected with my participation in this activity or my use of WAC equipment or facilities, including such claims which allege negligent acts or omissions of WAC. Should WAC be required to incur attorney’s fees and costs to enforce this agreement, I agree to indemnify and hold it harmless for all such reasonable attorney’s fees and costs. I also agree I shall be financially responsible for any expenses incurred or damages suffered by me or any other participant as a consequence of my personal acts or omissions while participating in this activity.

I certify that I have adequate health insurance funds for healthcare costs to cover any injury or damage I may suffer while participating in this activity. I further certify that I have no medical or physical conditions, which could interfere with my safety or the safety of others in this activity. I understand that WAC does not assume for or have an obligation to provide financial assistance or other assistance, including without limitation medical, health, or disability insurance of any nature in the event of an injury, illness or death, or damage to or loss of my property.

In the event that I file a lawsuit against WAC, I agree to do so solely in the State of Colorado, and I further agree that the substantive law of that state shall apply in the addiction without regard to the conflict of law rules of that state. I agree that if any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect. I fully realize and accept the responsibility to myself and to the opter participants to carry out all activities according to WAC procedures and in a safe and prudent manner.

I give my permission to use any images taken of me in any media purpose (except pornographic and defamatory) which may include, without limitation, advertising, promotion, marketing and packaging for any product or service.

I agree that I have had sufficient opportunity to read this entire document and to ask questions. I have read and understood it, and I agree to be bound by its terms.

TO BE READ BY PARENT/GUARDIAN OF MINOR I represent or warrant that a) I am the parent or legal guardian of the minor(s) named above, b) I have the legal right, power and authority to consent to this agreement on behalf of the minor(s) and myself c) I am at least 18 years of age. I have read and understand this entire agreement. By signing below, I hereby consent to approve in all respects the terms and conditions of this agreement, and agree that both the minor(s) and I shall be bound by all of the terms and conditions. I understand that this consent and approval is not revocable. I agree to defend, indemnify and hold harmless WAC from any and all claims, demands or causes of action which are in any way connected to the minor’s participation in this activity.
