Media Consent Form
I hereby grant full permission to York University, Regenesis, and the Green Career Centre for the collection and use of my personal images through photography or video recording, including audio, collectively referred to as "the Media."
I acknowledge that the Media may be utilized on York University, Regenesis, and the Green Career Centre's websites, in newsletters, publications, as well as shared with other members, staff, and affiliates. Furthermore, I understand that the Media may be employed by York University, Regenesis, and the Green Career Centre, its members, and affiliates for promotional purposes related to future events. I am aware that the Media may be disseminated widely and, if posted on York University, Regenesis, or the Green Career Centre's websites or other platforms, will be accessible to the public. I also recognize that while York University, Regenesis, and the Green Career Centre will take reasonable precautions to prevent unauthorized use of the Media, they cannot control what happens to it once it is accessible to the public.
For the aforementioned purposes, I consent to being photographed and/or recorded via video and/or audio by York University, Regenesis, and the Green Career Centre or their authorized representatives.
I also provide full permission to York University, Regenesis, and the Green Career Centre to use my name. I acknowledge that I will not receive any form of remuneration or payment for the film or photograph(s), either now or in the future.
Liability Waiver Form
In consideration of York University, the Green Career Centre, or Regenesis and their affiliated organizations ("Releasees") allowing me to participate in their programs, events, meetings, and any other gathering initiated by York University, the Green Career Centre, or Regenesis and their affiliated organizations, I agree as follows:
ASSUMPTION OF RISK
1.10 I ACKNOWLEDGE AND AGREE THAT participation in York University, the Green Career Centre or Regenesis and their affiliated organizations' programs, events, meetings, and any other gathering initiated by York University, the Green Career Centre, or Regenesis and their affiliated organizations involves potential dangers, risks, and hazards.
1.20 I FURTHER ACKNOWLEDGE AND AGREE THAT my participation in York University, the Green Career Centre or Regenesis and their affiliated organizations' programs, events, meetings, and any other gathering initiated by York University, the Green Career Centre, or Regenesis and their affiliated organizations is entirely at my own risk, and I freely accept all the inherent risks of participating in the programs, events, meetings, and any other gathering initiated by York University, the Green Career Centre, or Regenesis and their affiliated organizations and the possibility of personal injury, death, assault, property damage, and loss resulting therefrom.
RELEASE AND WAIVER OF CLAIM
I WAIVE ANY AND ALL claims I may now, and in the future, have against, and release and discharge from all liability, and agree not to sue, York University, the Green Career Centre, or Regenesis and their affiliated organizations, its officers, employees, agents, representatives, and each of them and their respective agents, administrators, representatives, heirs, successors, and assigns, with respect to any and all liability, costs (including legal costs), claims, damages, demands, actions, and causes of action of whatever kind which might arise from or in connection with my participation in the programs, events, meetings, and any other gathering initiated by York University, the Green Career Centre, or Regenesis and their affiliated organizations including, without limitation, any personal mental or physical injury, illness, death, property damage, loss of personal freedom, or financial loss or other loss suffered by me or any other family members or dependents, arising, directly or indirectly, from my participation in programs, events, meetings, and any other gathering initiated by York University, the Green Career Centre, or Regenesis and their affiliated organizations, whether foreseen or unforeseen and regardless of the cause thereof, including, without limitation, negligence or partial negligence on the part of the Releasees or any of them.
INDEMNITY
I AGREE to hold harmless and indemnify the Releasees for any and all claims made against any of the Releasees by any person, including any claim or action by or on behalf of my family or friends (present or future), for damages suffered or costs incurred arising out of or related to any aspect of my participation in the programs, events, meetings, and any other gathering initiated by York University, the Green Career Centre, or Regenesis and their affiliated organizations, including, without limitation, any of the matters described or contemplated at item 2 above.
UNDERSTANDING
I CONFIRM THAT I have read and understood this Agreement prior to signing it, and I agree that the legal rights and obligations under this Agreement will be binding upon my heirs, next-of-kin, executors, administrators, and successors. I am aware that by signing this Agreement, I am releasing and waiving certain legal rights, including the right to sue and to be awarded potentially substantial damages, which I or my heirs, next-of-kin, executors, administrators, and assigns have or may have against the Releasees.
JURISDICTION
I AGREE that this Agreement shall be governed in all respects by and interpreted in accordance with the laws in Canada, including applicable provisions of the Freedom of Information and Protection of Privacy Act (FIPPA).
