Please use this form to reserve a spot in the children's kumite class for yellow belts and above, ages 6-16, taking place on Saturday, August 6, beginning at 10:30am. Students must have kumite gear! Spots in this class are limited, and will be awarded on a first-come/first-served basis. All requests must be received by 5:00pm on Friday, August 5.

In order to be eligible to attend class, please check each box to verify your willingness to follow these requirements: *

Every student's and spectator's temperature must be taken within 30 minutes before leaving your home, and must not exceed 100.1* farenheit

You agree not to attend class if you or anyone in your household is showing any symptoms that may be related to COVID-19, including fever, cough, or loss of taste/smell.

You agree not to attend class if you or anyone in your household has been exposed to anyone who has COVID-19 or is showing any symptoms that may be related to COVID-19.

You agree not to attend class if you or anyone in your household has traveled outside of the United States and has not completed the required quarantine and/or testing.

Students and parents must wear a mask or cloth face covering

Proper physical distance must be maintained at all times

Parents/Spectators are limited to 1 per student.
