Please fill out the absence request form below. This form must be completed and sent in PRIOR to an absence.  You will be contacted if the absence is not approved. Each company member is allowed 6 absences for the season (August through June). An absence due to illness does not count towards your total number of absences.

Please note that dancers are responsible to learn any new choreography or notes missed from another dancer BEFORE the next scheduled rehearsal!
