(2023-24) Fields Institute - School Visit Request Form
Each year the Department of Mathematics opens it doors to schools and home-schoolers* that wish to visit us at the University of Toronto Campus. We provide various lectures, hands-on workshops, and other mathematics based activities for a wide range of grades/student groups.

This form will be used as a basis to book your school visit at the University of Toronto. However, please note that simply filling out this form does not confirm a visit for your group. Once we have received your request, someone from our office will contact you to clarify and confirm your visit details. During our busy periods it may take several weeks before you receive a reply.

*Please note that we cannot offer workshops for a single family of home-schooled children. Home-schoolers interested in a math field trip are asked to coordinate with other families to create a group of at least 10 children within three-grade range. If you are looking for mathematics enrichment for individual home-schooled children, we highly recommend our enrichment courses and math contest training. We currently offer a 75 minute workshop that runs from either 10:30am - 11:45am or from 1:15pm - 2:30pm.

Because of the nature of our programs we ask that you to provide us with at least three possible dates for your visit and we will try to accommodate as best as possible.  In the event we are unable to provide you with any of the requested dates a member of our staff will contact you to discuss options.

The grade level is suggested, but you may sign your class up for whichever topic you are interested in. We will try our best to find a facilitator that can lead your chosen topic, but *please note that this does not guarantee your chosen topic.* Some facilitators even bring their own new topics that are not on this list.
