By checking this box you understand that 360PartyBaltimore (operated by JJonesDigital LLC) is contracted for the time stated in this questionnaire. The 360PartyBaltimore (and JJonesDigital LLC)is not responsible for your event starting late or guests showing up in a timely manner. If you want 360 Party Baltimore to stay additional time after what is stated in this questionnaire there will be an additional cost if time is available.

By checking this box you understand that if you or any of your guests damage any of the equipment brought by 360PartyBaltimore (and JJonesDigital LLC). You are 100% responsible for the cost of the damaged equipment, along with the balance for the event.

By checking this box you understand that 360PartyBaltimore (operated by JJonesDigital LLC) is not responsible for any weather conditions that may arise the day of your event if outdoors, by which we have to stop our services. If this happens you are responsible for the balance if we cannot move indoors to continue service.

WAIVER OF LIABILITY: Participant hereby forever releases and waives his or her rights to bring suit against the 360PartyBaltimore or JJonesDigital Photography, LLC and its owners, officers, directors, managers, agents, employees, contractors, operators or other representatives in connection with exposure, infection, or spread of COVID-19. Client understands this waiver means that s/he gives up his/her right to bring any claims including personal injury, death, disease, property or monetary loss, or any other loss, including but not limited to negligence, and gives up any claim s/he may have to seek damages, whether known or unknown.

INDEMNIFICATION: Client agrees to hold harmless and indemnify 360PartyBaltimore or JJonesDigital Photography, LLC against all third-party claims, damages, liabilities, losses, suits, costs, expenses and attorney fees suffered or incurred by 360PartyBaltimore or JJonesDigital Photography, LLC and arising from 360Party Baltimore or JJonesDigital Photography, LLC participation in the event.

CHOICE OF LAW: Client agrees that the law of the State in which the event is held will apply to this contract.

I have carefully read and fully understand this waiver and release and freely and knowingly assume the risk, waive my rights concerning liability as stated above and agree to all the terms. My participation acknowledges that I agree and have read these terms.
