GFS Athletics Pre-Participation COVID Questionnaire
It is important that all athletes return to sports safely this fall. This is particularly true with those who have tested positive for COVID-19. Most young people who test positive with COVID-19 have mild symptoms or remain asymptomatic. However, some people have been shown to develop cardio-pulmonary complications as a result.

All students wishing to participate in on-campus athletics must submit this survey. If you answer "yes" to any of the questions below, a medical clearance for participation in physical activity must be submitted to the Garrison Forest School Athletic Trainer, Barbara Doy.  Barbara Doy can be reached at barbaradoy@gfs.org.
