All current or prospective members of Yate Hockey Club are required to complete this registration
form and return it, completely filled in, every season.

All information you provide to us is stored in secure, password protected databases. The Club shall
make all reasonable efforts to hold data only electronically and in a structured form e.g. on a
password protected database. All other forms of unstructured data holding need to be kept to a
minimum e.g. keeping hard copies, keeping files stored on personal devices. The aim is to make it as
easy as possible for the Club to identify, locate, and control data held by the Club. For more
