Sign up for the same time every week or different times and days depending on your schedule.  If you're selecting the same time every week and you need to miss, please mark those dates down in the comment section at the end of this form, so that I can plan accordingly.  

Remember, only students that take lessons over the summer are guaranteed first choice of lesson time spots in the fall!  

Please note that there will be no lessons August 12 - 16.
