Westhampton UMC Child Protection Policy Form
This form keeps Westhampton United Methodist Church up to date on our children's needs, our Workers (volunteers and WUMC employees), and provides basic protections to the church.  

This form is broken into two parts:

1.  Worker information
2.  Child/Youth Participant information

Per our Child Protection Policy, a "Worker" is defined as " any adult who serves as a volunteer and/or paid person given the responsibility of working with or caring for minors."  

Examples of workers at Westhampton include but are not limited to: nursery volunteers, head ushers, Christmas pageant helpers, VBS decorators, Trustee workday volunteers, etc.
